School Improvement Plan
2025-26
Goal 1
By June 2026, Yorktown Middle School will increase the percentage of students with disabilities achieving proficiency in reading from 47% to at least 65%, and in math from 41% to at least 60%, thereby significantly narrowing the achievement gap compared to overall student performance (currently 79% in reading and 72% in math).
Goal 2
By June 2026, each PLC at YMS will implement and document two targeted instructional strategies per core subject (Reading, Math, History, Science) to support students with disabilities (SWD). Initial strategies include:
- Small Group Instruction based on student data.
- Explicit Vocabulary Instruction embedded in daily lessons.
PLCs will meet biweekly to review student progress and adjust instruction using formative assessment data.
Goal 3
By June 2026, no more than 10% of Yorktown Middle School students will be chronically absent (missing 10% or more of the school year). 75% of students will be connected to school through at least one club, sport, or co-curricular activity.
Goal 4
By June 2026, the school will reduce office discipline referrals from a baseline of 339 in 2025 by 25%, by strengthening PBIS practices, increasing staff consistency in behavior management, and implementing targeted Tier 2 and Tier 3 student supports. (*Goal changed from original goal based on office referrals instead of the risk ratio)
